Research Focus

My research focuses on improving the quality, equity and coordination of care for older adults and vulnerable populations across the healthcare continuum. I study models of transitional and post-acute care that strengthen communication between hospitals, primary care and community services to optimize patient outcomes and system efficiency. My work also explores how digital health and artificial intelligence can be designed and implemented in ways that promote equity and inclusion in health systems. Through interdisciplinary collaborations, I contribute to projects addressing complex care transitions across the lifespan, long-term care innovation and the integration of emerging technologies to support aging and other vulnerable populations. My research is closely linked to clinical practice and health system leadership, with the goal of translating evidence into practical solutions that improve patient care and safety and support more inclusive, responsive and cost-effective health systems. I have also contributed to the development of key educational initiatives, including the Black Health Primer Course, Supporting Cancer Survivors Return to Work: an online course for family practitioners, and the MUHC Cancer Care Transition of Care Program.

I am currently the Director of the Secondary Care Division in the Department of Family Medicine at the MUHC. I also serve as Medical Director and Service Chief for NSA, translational and long-term care services at the Montreal General Hospital.
